examples/ip_fragmentation: overhaul

New stuff:
* Support for regular traffic as well as IPv4 and IPv6
* Simplified config
* Routing table printed out on start
* Uses LPM/LPM6 for lookup
* Unmatched traffic is sent to the originating port
