net/mlx5: remove Verbs dependency in spawn struct

1. Replace 'struct ibv_device *' with 'void *' in 'struct
mlx5_dev_spawn_data'. Define a getter function to retrieve the
device name.
2. Rename ibv_dev and ibv_port as phys_dev and phys_port
respectively.
